**Second Dose Booster Shot**
You are eligible for a 2nd booster dose if it has been ≥ 4 months since your FIRST booster dose of any authorized vaccine, and meet one of the following requirements below:
- Everyone ≥ 50 years old can receive a 2nd booster of Moderna or Pfizer
- ≥ 12 years old + have a weakened immune system, you are eligible for a 2nd booster dose of Pfizer vaccine.
- ≥ 18 years old + have a weakened immune system, you are eligible for a 2nd booster dose of Moderna vaccine
Pfizer 1st & 2nd dose for Patients 5 – 11 years AND Booster for 5 - 11 years
- NOT ALL PHARMACY LOCATIONS ARE ADMINISTERING PFIZER IN THIS AGE GROUP
- When scheduling, you MUST select “Covid-19 Vaccination 1st Dose Pfizer **5-11 YEARS OLD ONLY**” or Covid-19 Vaccination **1ST** Booster ** 5-11 YEARS OLD ONLY ** Pfizer
- Currently, Quality Care Pharmacy and Albert's Pharmacy are the only pharmacies administering Pfizer to 5 – 11 year olds
1st & 2nd dose COVID-19 vaccines are recommended for all according to the following:
- Moderna or J&J: Must be 18 years of age and older
- Pfizer: Must be 5 years of age and older
**First Dose Booster Shot**
Pfizer and Moderna
- Anyone ≥ 12 years can receive a single COVID-19 vaccine booster dose ≥ 5 months after completion of their primary mRNA vaccine series
Janssen (J&J) Boosters
- Anyone ≥ 18 years can receive a single COVID-19 vaccine booster dose ≥ 2 months after their first Janssen (J&J) dose
3rd Dose (Immunocompromised ONLY) – A 3rd dose of Moderna or Pfizer COVID-19 vaccine ONLY if the following criteria are met
- People that are moderately to severely immunocompromised according to the CDC (Click here for criteria)
- 3rd dose is at least 28 days after your 2nd dose of Moderna or Pfizer
You must bring a valid government-issued ID with your appointment in order to be vaccinated. If <16 years old you can bring a passport, birth certificate, or medical document with name and DOB on it. We are not able to vaccinate without proper identification that matches your appointment confirmation.
IMPORTANT:

the patient is < 18 years old, a parent or legal guardian MUST BE PRESENT at
the time of vaccination***
